CCI Receives NCQA Case Management Accreditation

GREENVILLE, S.C. – The Care Coordination Institute (CCI) has received Case Management Accreditation from the National Committee for Quality Assurance (NCQA) for its care management program and a Certification for Disease Management for its condition management program. Earning NCQA’s case management accreditation is an indication that a Case Management program is dedicated to care coordination, patient-centeredness and improving quality of care: getting the right care to patients.

CCI’s care management and condition management programs are designed to drive patient engagement and coordinate care across the continuum. By leveraging robust risk categorizations paired with evidence-based best practices, CCI’s programs ensure that the right care is delivered in the right setting through care plan design, implementation and monitoring. CCI’s continuum-based approach leads to improved health outcomes, positive patient experiences and reduced overall cost of care.

Care management services are provided to patients who have experienced an event or diagnosis requiring the extensive use of resources and who need help navigating the system to facilitate appropriate delivery of care and services. It is a collaborative process to meet the comprehensive medical, behavioral health, social, and spiritual needs of a patient and the patient’s family or caregiver, while promoting quality and cost-effective outcomes.

Condition management services are provided for all eligible patients identified with diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), asthma, hypertension and hyperlipidemia. The program emphasizes prevention of exacerbations and complications utilizing evidence-based practice guidelines and a collaborative process that identifies opportunities for prevention or intervention.

NCQA is a private, non-profit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations.
